Immunization Updates
Per Arizona Law, if a student’s immunizations are not current, s/he may not attend school. For a detailed listing of school immunizations, please refer to the following website. - read now
Arizona law requires the following immunizations.
• Td: (tetanus) is required every 10 years
(** see below for new guidelines)
• MMR – 2 total immunizations are required ( the first at or after 12 months of age, and the 2nd must be at minimum of 28 days later.
• Polio – 4 total doses are required unless the child was over the age of 4 when the 3rd dose was given. The doses must be at least 4 weeks apart.
• Hepatitis B – 3 total doses are required. Dose #1 and #2 must be a minimum of 4 weeks apart and dose #3 must be at least 8 weeks after dose #2. Also, dose #1 and #3 must be a minimum of 16 weeks apart.
• Varicella (Chicken Pox) – Currently 1 dose is required for any student entering high school that has not had Chicken Pox disease. If your student has had the disease, you must complete the varicella form.
